I had an old green book provisional license, which I lost! Can anyone tell me whether it is a form you can request a provisional license fully online again?

You must obtain an application form D1 from any post office. You also need a passport photo in color, identification, and 22 pounds. The ID is preferably a passport.
If you have a passport that was issued in the last 8 years have, then it will be digital (one can say whether the digital photograph and signature on the same page) and everything you need to do is write your passport number in the form of D1 and nobody has to sign his photo.
If your passport is not a number (in the photo and signature) on the next page, you can be your passport with your photo and shape to a post office where you pay an extra £ 4 will be the passport control and then back again. They have no one to sign the photo.
Alternatively, you can use the passport to the DVLA.
If you can not send a valid passport, one day, if not more than 5 years, or a birth certificate expired in the United Kingdom. If you are married, you will need your marriage certificate.
If you have another form of identity OTher that a passport for your signed photo and article 7 of the D1 form from someone who has been known for over 2 years of completion is required, not a parent not living with you.
